Veiling Opacity of the Left Hemithorax from Pleural Effusion Supine portable AP chest X-ray demonstrates a diffuse, hazy increase in density across the entire left hemithorax, consistent with a “veiling opacity.” This finding obscures the normal pulmonary vascular markings and the contour of the left hemidiaphragm. This “veiling opacity” is a classic sign of a pleural effusion on a supine radiograph. In the supine position, mobile pleural fluid flows under the effects of gravity and layers posteriorly. This sheet of fluid creates a diffuse increase in density as the X-ray beam passes through it, rather than forming the sharp meniscal sign seen on an upright film. {Citations needed} On a supine CXR, a pleural effusion layers posteriorly, resulting in a diffuse “veiling opacity” of the affected hemithorax.
